Our Independence packages support young people in moving on from their Care in Mind placement into the next step of their recovery journey.
These packages are provided as a satellite provision to the Care in Mind home, into an individual tenancy.
Delivered in phases over a period of up to 12 months, our teams offer a gradually reducing package of care to support young people in making the transition from their residential placement to independent life in the community.
Young Person profile
- Age 16-30
- Has been placed in a Care in Mind residential home and is ready to move on to the next step
- Has demonstrated meaningful engagement in their care plans and therapeutic interventions
- Is wanting to remain within the locality in which they’ve been placed
What we offer
- A three-phase step-down programme delivered into independent placements in the community, by a known, consistent staff team from their Care in Mind placement
- Continuity of care, bridging the gap between residential living and independence
- Graded reduction in MDT support via a structured approach
- Phase 1: Stablisation in the community
- Phase 2: Care delivery and community intervention
- Phase 3: Close down and handover to community services
