Our specialist residential homes support young people with complex mental health needs in intensive MDT supported placements in the community.
Through a trauma-informed, least restrictive model of care, underpinned by a therapeutic risk management approach, we support young people in achieving their recovery goals and moving on into independence.
We aim to empower young people to achieve their fullest potential, through high quality, person-centred care.
Young Person profile
- Age 16-30
- Complex mental health presentation
- Diagnoses may include Personality Disorder, ASD or neurodiversity, Complex PTSD, Attachment Disorder
- Significant history of risk to self
- May be in an inpatient mental health setting, Tier 4 CAMHS provision or secure service approaching readiness for discharge
- Would benefit from a robust, planned transition
What we offer
- Comprehensive pre-admission assessment by a mental health professional to develop a full Assessment of Needs and Prescribed Interventions
- Collaborative working with current placement and discharging teams to design an individually tailored transition plan
- Management of complex needs including risk to self therapeutically to achieve goals towards recovery
- Managed transitions including the development of relationally focused, robust care plans to safely transition between placements; essential where young people’s risks have been managed for a long time through more environmentally restrictive environments
- Fully integrated support from a comprehensive MDT offering direct and systemic intervention throughout placement
