Our Rapid Intake and Assessment pathway is tailored for young people who may have no stable base to support a planned transition.

They may find themselves in situations of heightened risk, or loss of previous placement, and returning home may not be an option.

We offer a rapid assessment and swift admission to one of our CQC registered care homes. During a 12 week assessment and stabilisation programme our comprehensive wraparound MDT undertake an extended assessment and develop a comprehensive needs assessment including future placement recommendations.

Following the 12 week programme, onward placement recommendations may include;

  • Return to the family home or previous placement
  • A longer term placement with Care in Mind to work on recovery goals
  • Transition to a more intensive service if indicated
  • Transition into independence with input from local teams
