Our Rapid Intake and Assessment pathway is tailored for young people who may have no stable base to support a planned transition.
They may find themselves in situations of heightened risk, or loss of previous placement, and returning home may not be an option.
We offer a rapid assessment and swift admission to one of our CQC registered care homes. During a 12 week assessment and stabilisation programme our comprehensive wraparound MDT undertake an extended assessment and develop a comprehensive needs assessment including future placement recommendations.
Following the 12 week programme, onward placement recommendations may include;
- Return to the family home or previous placement
- A longer term placement with Care in Mind to work on recovery goals
- Transition to a more intensive service if indicated
- Transition into independence with input from local teams
Young Person profile
- Age 16-30
- May be in crisis following community placement breakdown or following admission to a medical/paediatric ward with no identified placement to return to
- No stable base to support a planned transition
- There may be a lack of clarity around diagnosis and formulation of risk
- Requiring a period of stabilisation and assessment

- Three month stabilisation programme that includes robust assessment, formulation, evidence-based interventions and care planning
- Production of a comprehensive needs assessment including placement recommendations and a transitional plan
