We provide a specialist team to deliver intensive support to inform and facilitate discharge planning, support admission avoidance or enhance placement sustainability in community settings. Our Reach Team is comprised of specialist clinicians and highly trained support workers.
The Reach Team bridges the gap between health and social care providing continuity of approach for young people, their families and carers.
Our head office, Hope House in Stockport, serves as the central hub for our regional clinical teams — supporting both those working within residential provision and those delivering alternative clinical packages. You can view our most recent CQC rating for these services here:
What we offer
- In-Reach into existing placements or home environment to support:
- Admission avoidance where hospitalisation is contra-indicated
- Placement sustainability where complexity or risk may be increasing but admission thresholds are not met, to provide systemic support to prevent placement breakdown and repeated placement moves
- Supported Transitions into and out of placements to provide relational security to bridge the gap between provisions
- Out-Reach support with move on to independent living post-placement; reducing package of care over time to facilitate successful step-down and stabilisation in the community
