Since 2012, Care in Mind have been offering specialist mental health services to young people aged 16-30 presenting with high levels of complexity and risk.
Many of the young people we support in our specialist residential mental healthcare placements transition to us from inpatient units and may have experienced long or numerous previous hospital admissions and/or placement breakdowns.
Our innovative and evidence-based model of care aims to support young people in managing their risky behaviours collaboratively, using least restrictive trauma-informed approaches. We encourage them to engage in meaningful therapeutic programmes and work toward independent life in the community.
Our consultant led multidisciplinary mental health teams provide expert support and therapeutic intervention as well as 24/7 on-call and crisis intervention support, which supports our young people in maintaining their community placement and avoiding readmission.
We pride ourselves on being a collaborative partner for young people, their families, local authorities and the NHS. Our approach ensures that the care we provide is well-integrated and consistent with broader treatment plans. From the point of referral through to discharge and transition planning, we maintain close communication with professionals and families to ensure each young person receives the highest standard of care.
Our outcomes demonstrate that our model works in helping young people to sustain placements, reduce their risks and move toward discharge to more independent living.
